Notification Listing
Guidance is available for operators planning to submit notifications for use of
"other technology" to perform IM assessments. This includes
guidance for submission
drawn from Advisory Bulletin 05-04, dated July 29, 2005, and a
review checklist
describing considerations used by PHMSA in its review of notifications of this type.

Methods of Submittal
As identified in §192.951 an operator must provide any notifications by -
- Sending the notification to the Information Resources Manager, U.S. Department of Transportation, Pipeline and Hazardous Materials Safety Administration, East Building, 2nd Floor, 1200 New Jersey Avenue SE, Washington DC 205901;
- Sending the notification to the Information Resources Manager by facsimile to (202) 366-36661; or
- Entering the information directly on the Integrity Management Database (IMDB) web site at http://primis.phmsa.dot.gov/gasimp/ (this site).

These options are all available today. The web-based upload option is the most expedient.
To use the web-based option, first download and complete the
Microsoft Word template (see below)
and use it to create a Word document (DOC file) for each Notification you are submitting. Then
use the upload/submission form to submit it to OPS. Do not upload draft submissions. The upload form immediately submits your document for OPS review!

Note: Operators must contact individual states for notifications also required to be submitted to state and local
authorities (i.e., where the state is an agreement state or the affected pipeline is an intrastate line under
state jurisdiction). Notifying OPS, by any of the three methods, does not satisfy the requirement that operators
also notify states in these circumstances.
How To: Download and Use the Notification Template
To facilitate notifying OPS, a
Microsoft Word template
has been created.
Caution:
Do not open the template by directly following the link below! Depending on your browser version,
this could cause Word to open within your browser context, which can cause your work to be easily lost!
Instead, we recommend the steps below.
- Save the template file (.dot file extension) to a local file on your computer. To do this depends
on your browser, but most common is to right-click the link and choose "Save Target As.." (MSIE) or
"Save Link As..." (FireFox).
- When saving the file, choose a file location where you can easily find the file. If you use other Word templates,
one convenient location is the folder specified for "templates" under the "File Locations" tab under Word's Options.

- Once saved, you can create a new Word document from that template, by either double-clicking the template file, or
using the "New.." choice from the File Menu and selecting the template as the basis for the file.
- Complete the notification using the instructions right within the document.
- Save the completed document as a regular Word document file (.doc), using the file naming suggestions at the
end of the document.
- Submit the completed document using the upload/submission form. Complete the information requested on that form, attach the file using the File Attachment and Additional Comments section. Click Save and your notification will be submitted automatically.
- You may also mail or fax your completed document as described above.
Repeat steps 3-5 for each notification you submit! Do not combine different types/topics within a single notification.
