This page is for use by operators in submitting notifications to PHMSA as required by Part 192.949 and related paragraphs.

NOTE: In addition to notifying PHMSA, operators must contact individual states for notifications also required to be submitted to state and local authorities!

Prior to using this page, you should prepare your notification and complete any internal reviews. The preferred approach is to use the Word template and upload a completed document. The Word template is available on this site.
