Pipeline Safety Connects Us All

All Reported Pipeline Incidents

This data set contains all incidents which were reported to PHMSA based on reporting criteria that was in effect at the time of the incident and which has changed both significantly and multiple times over the years. For specific information and dates concerning these changes between 1990 and 2011, see History of PHMSA Incident Reporting Criteria.

It should also be noted that the All Reported Incidents data set does not include any adjustments to account for commodity cost fluctuations or general inflation as has been done in the case of Significant Incidents. As a result, the All Reported Incidents data set is not intended for historical trending. For the data set which is intended for historical trending, see Significant Pipeline Incidents.

The tables below show the number of incidents reported in a year for each type of pipeline operator. Each year and selected column totals provide links to focused reports showing the causes of the corresponding incidents.

The data source for this table is the PHMSA Flagged Incident File.(1)

Where appropriate, the table columns can be sorted by clicking the corresponding column header.

In addition to the Significant Incident designation mentioned above, there is also a designation for Serious Incident which counts only incidents involving a fatality or injury. See Serious Pipeline Incidents for information on this smaller subset of incidents.

Notes

  1. The costs presented here are nominal (that is, as reported by the Operator at the time of the incident) and are not adjusted to account for commodity cost fluctuations or general inflation.
  2. For years 2002 and later, property damage is estimated as the sum of all public and private costs reported in the 30-day incident report. For years prior to 2002, accident report forms did not include a breakdown of public and private costs so property damage for these years is the reported total property damage field in the report.
  3. Net Barrels Lost applies only to Liquid incidents and is the difference between Gross Barrels Spilled and Barrels Recovered.
  4. Does not include 1,851 injuries that required medical treatment reported for the October, 1994 accidents that were caused by severe flooding near Houston, Texas.
  5. In 2002, one of the more significant of several incident reporting criterion changes occurred. One of PHMSA's reporting criterion for hazardous liquid pipeline incidents (the one based on volume released) was lowered from 50 barrels to 5 gallons, resulting in a significant increase in the number of hazardous liquid incidents reported.
  6. Approximately 94% of the Property Damage for Gas Distribution Incidents in 2005 was caused by flooding in New Orleans. The $466.5 million associated with this incident represents lost gas and operator property damage, but does not include damage to public and private property caused by the flooding.
